Patients must present their current insurance card to the receptionist before
every visit. Please notify us as soon as possible of any change to your
insurance carrier and/or contact information.

Payment:
Payments must be made at the time of service. All patients whose insurance
contracts include a copayment are required to render this payment at each
office visit. We accept cash, check and most major credit cards.
